Users can't tag their own threads here. Tags have to be added by the mods.
As for your other problem, where's your .pk3, .wad, or .zip? (I don't have any mods that are ready to submit…. well, maybe one very simple mod… the point is, I don't have any direct experience with the addon submission form to help you with.)
