Got it, Had to push a horse into the diner to open the well. Awesome how both of you were correct! Thanks for the help!
Oddly, It looks like its cut short a couple seconds, i can only speculate why, Perhaps this was intended to be the original end point? with you falling down the hole? This would make sense with the other secret map endings... Additionally The water has no kill or hurt flag things, overall the conclusion here is that this secret stage was probably rushed for some reason. though i have no idea why.
