Here's the basic gist of it: Things and Linedefs with No Easy/No Normal/No Hard flags checked will convert properly with regards to difficulty, with selectable difficulty as another command line argument (1=easy, 2=normal, 3=hard). As usual, removed things are changed to thing type 4242, which is secretly thing type 146. Linedef effects are also changed to type 4242, except that's actually 4242. The Linedefs will also drop their difficulty flags so they don't end up doing weird stuff.
MISC:
Floor spikes will get their deaf flag checked, turning them solid.
Inferno shields convert to Elemental.
