Perhaps the guest logged in to check his private messages, and it didn't update properly? Keep in mind, the guest view isn't entirely accurate, as it displays multiple actions from a single IP, based on my experience with the Administrator Control Panel.

This is a very old glitch in PHPBB. Either what Foxblitzz said is true, or they clicked the "Log In to view Private Messages" link, and that's how it displays.
