The scariest games I've played is:

Silent Hill 2
Resident evil 4
Condemned Criminal origins
The suffering
and F.E.A.R(Project Origin)

FEAR: Extraction Point.

Found it far more scary than the original game.

Oh, and Jackthesilent, Project Origin is the name of FEAR's sequel, not FEAR.

Project Zero 1/Fatal Frame 1.
I can't stand that game...so...freaking...scary.
Also another one is Silent Hill 4, though I can play the others without a sweat, the ghosts are just...disturbing.

Alien Vs. Predator 2 scared me at the time. Ravenholm in Half-Life 2 gave me chills the first time I played through it.

Nothing new at the moment, however.

Ecco the Dolphin... it terrified me when I was little, and playing through some of the levels (or even viewing speedruns) is something I still find somewhat... 'scary' for lack of a better term.
