1. Yes. The palette you need to use is in srb2.srb. If your IWAD for your lump editor is set to srb2.srb(the 2.0 one) They'll be auto converted to the palette when imported.
2. No. Not really. But we don't want a 200MB char now do we?
 
The Doom graphic format doesn't like more than 255 rows of pixels, so there's a size limit right there.
