Problem with inserting map header

I also have a problem - whit inserting the Map Header. I dont`t know what to write for:
File name to add:
Final lump name:
Lump to add after:

I don't understand where you're getting the idea you have to type those things. The map header guide tells you exactly what to type, use that.

Yes , I read that and I did everything it said . I type the wad name, Action to take :(I type ) add
then it sais
File name to add:(I type ) MAP01D map01d.txt
then it sais
Final lump name: I have no idea what to type here.
then it sais
Lump to add after: ?????

Use the version of LumpMod from http://www.lawrencegearheadz.org/~a441/files/lumpmod.zip

If you have a lumpmod.bat, that's something someone else made. Delete it, and the instructions should work.
