cmcs21.ocx is included in the SRB2DB installer (but not the patch). Reinstalling SRB2DB should fix it. If, after doing that, it still doesn't work, run the following at the command-line:

regsvr32 cmcs21.ocx

Apparently you downloaded the patch to upgrade SRB2 Doom Builder, but not SRB2 Doom Builder itself.

Try downloading the file from here and then doing the same to it as you did with MSCOMCTL.OCX, i.e. putting it in your system32 directory and registering it.
