In THE MUSICDEF file, you exclude the 'O_' part of the Lump. So, if you were trying to add something like 'O__TEST.ogg' into the jukebox, you'd add it by putting 'LUMP _TEST' into the MUSICDEF file. Does the folder where SRB2 is located have a semi-colon in the name? If so, then that's why, as SRB2's addons menu and addfile command doesn't like it if addons or even SRB2's home folder has a semi-colon in the name.
