Milkshape, 3DSMax, Blender, pretty much any model editing software works provided it has animation support, and either MD2 support native, or a plugin that adds MD2 support.

Personally, I think Misfit 3d works best with MD2's. You need to check it off in the installation.

Yes it can. If you feel like wasting money on Sketchup pro. In my opinion, Sketchup is the easiest to use, but the least powerful by far.
