Why yes, a chandelier would fit in nicely... although I have no idea how that would be made in SRB2.
Middletextures with transparent pixels, intersecting with each other. A few circles depicting the candle-holding rings, and an * of chandelier tresses/hanging cables to intersect the rings, and you can get a pretty good result. Line up some thing chains to hang it from the ceiling and use an invisible FOF or two for collision, and you can place it just about anywhere!

It goes without saying that you have to make the textures specifically for the geometry you come up with, though. Vanilla SRB2 doesn't have anything you can just slot in for that.

I was thinking, on the same texture as the chain, would it be better to have the flame pictured there too? It would be like how flowers on Minecraft have two textures intersecting each other to create the object. I think it'd look something like this (but much, MUCH better in appearance):

My apologies if it seems like I'm going too far here. I just thought it'd be worth making this since I wanted to get the idea across.
 
Honestly, I'd make the flames objects. They would look the same from any angle, and you'd be able to give them fullbright.
 
Flames should probably be mobjs, yes, though the intersecting thing could be used for the sconces. Don't forget the tresses in the intersecting lines, though.
 
Thanks for the suggestions, but... I'm not really considering a chandelier. Or, at the very least, not one composed primarily of midtextures. The trouble with trying to emulate a "real" casino is that those kind don't have hedgehogs running through them, and the Sonic levels that do don't have very obvious ceiling lighting. I'm going to look into alternate lighting solutions that are suitably Casino-esque without being ridiculously ornate/distracting. I'm possibly thinking some sort of neon, cylindrical lantern, whether embedded in the ceiling, floor, or even floating in midair.

Also, that ceiling is the floor for an alternate path, so.
 
How did you do that? Is each spinning frame a different patch entirely?

One thing I think you should do is make the flashing animation a bit more noticeable. Not saying you should make it seizure-inducing, but I could hardly tell that it was flashing. Another thing that kind of bugs me is how bright the white background is... perhaps it's just the small GIF size making the icons harder to see. It would look lovely with some shading to make it look "round," I suppose, or like wheels rather than flat panels. Regardless, I love it. Can't wait.

also pls pls pls have it play the slot machine sound from casino night zone, I beg you
 
Each quarter of the slot machine's roller is a seperate graphic. Between 8 and 5 graphics are stored in a slots table at a time (dipping below this causes it to be reset to 8), with only the bottom 4 rendered. There's room for improvement in this part of the implementation. In addition, the flashing is a liiiitle more obvious at higher resolutions, instead of at a tiny GIF which takes up a small portion of your screen. Also, shading to make it rounder... was actually in the gif you looked at, but wasn't working very well so I took it out.

In other news, I got the visual element of the reward mechanism working! Woo.
 
They're HUD graphics alone! Physical objects wouldn't be able to make their way through all the level geometry, and would probably lag out the stage (plus mess with people when playing online).

In all honesty, all of this look awesome. Are those Rings and Spike Balls a bunch of standalones objects just shrinking down the screen? Looks very impressive tbh! All you need left for the pinball itself is actually some fancier decorations, because the actual generic black one looks kinda bland IMO.

I'm really impressed, I can't wait to play this stage! Is all this gonna work in OpenGL?
