I can't report a server.

I'm trying to report a server but the forms say,
Form failed to submit. The following error(s) occured:
You must upload at least one attachment to submit this entry. If you are submitting an addon, it may be necessary to split the music into a separate WAD file to enable you to upload it within out attachment limits. Click here to return to the previous page.

I have a zip file uploaded and it wont allow me to upload the wad itself. Can anyone help me?
 
Did the server mess with your srb2 in any way? was the server called nightmare by any chance?
