Agreed, these names only apply to generic level themes and most really interesting level themes should have some sort of twist whether it is a crossover between two themes, a different take on an existing theme, or anything like that. In that case the creator of the map would not be able to use one of those, and probably wouldn't be able to take splice level name suggestions together.
As for a template, try sketching a scene or two on paper, and use that for a template. It may not be a layout but it would give you that template you are asking for and even better than that.
Also when I come up with a name, as a general rule I want words included to be descriptive of the level, sometimes vivid, but never strange to the player, if he doesn't know what the word means it can detract from the familiarity of the level.
