Server Join Glitch

This happens to me a lot: when I try to join a server, somehow, the game mistakes me for another player and causing me to see the other players screen, extreme lag and synch failure. This also makes some servers crash. This happens constantly or maybe just once. It's very random. This only happens if someone joins the server, obviously. This can happen to ANYONE.
