Pretty sure that is intended from the beginning. The mushroom will be your only item until someone joins your game and then you will start to get different items. Think of it more as a time trial option since the single player option was removed.

The only wads that people are not required to have when they join a net game are music wads. Character wads, level wads and socs (I believe) are required if the host has them added to the server. And like Steel said, you can up your maxsend or upload it to 4shared or another file hosting site...

Did you forward your ports? You have to do this in order for anyone join your server. Of course, you could also consider looking at the wiki for more information and troubleshooting tips.
